Employment Type: Full-Time, Part-Time, or PRN
Job Overview:
A Hug Away Healthcare, Inc. is seeking a compassionate and experienced Physical Therapist (PT) to join our growing Home Health team. The Physical Therapist will provide coordinated, patient-centered care in the home setting, helping patients restore, maintain, and improve mobility, function, and overall quality of life.
The ideal candidate is a licensed Physical Therapist with strong clinical skills, excellent communication abilities, and a commitment to delivering exceptional care to patients and their families.
Responsibilities:
  • Perform comprehensive physical therapy evaluations and ongoing patient assessments, including OASIS assessments as applicable.
  • Develop, implement, and revise individualized plans of care based on physician orders and patient needs.
  • Provide skilled physical therapy interventions to patients with neurological, orthopedic, neuromuscular, and functional impairments.
  • Educate patients, families, and caregivers regarding treatment plans, exercises, safety precautions, and use of assistive devices.
  • Instruct patients in safe transfer techniques and the proper use of walkers, wheelchairs, canes, and other durable medical equipment.
  • Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care.
  • Maintain accurate and timely clinical documentation, progress notes, and physician communication.
  • Supervise Physical Therapy Assistants and Home Health Aides when appropriate.
  • Follow infection control standards and agency policies to ensure patient safety.
  • Participate in Quality Assurance and Performance Improvement (QAPI) activities and agency-sponsored training programs.
  • Promote the agency's mission, values, and commitment to quality patient care.
Qualifications:
Education:
  • Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited American Physical Therapy Association (APTA) program.
Licensure:
  • Current and active Texas Physical Therapist license (or compact license eligible for Texas practice).
Experience:
  • Minimum of two (2) years of experience in acute care, rehabilitation, or home health preferred.
Skills & Requirements:
  • Strong clinical assessment and therapy skills.
  • Knowledge of home health regulations and documentation requirements.
  • Experience with OASIS assessments preferred.
  • Excellent interpersonal and communication skills.
  • Current CPR certification.
  • Knowledge of durable medical equipment and mobility aids.
  • Ability to work independently and manage patient schedules effectively.
Additional Requirements:
  • Valid driver's license.
  • Reliable transportation.
  • Current auto liability insurance.
  • Ability to travel throughout assigned service areas.
Physical Requirements:
