Conscious Puzzles Mental Health Services is seeking an experienced, compassionate, and highly motivated Lead Care Manager to oversee our expanding CalAIM Enhanced Care Management (ECM) program through Partnership HealthPlan of California.

This leadership position is ideal for an individual with extensive experience in care management, community-based services, behavioral health, and interdisciplinary collaboration. The Lead Care Manager will provide program oversight, support care management staff, coordinate services for high-risk members, and ensure compliance with CalAIM and Partnership HealthPlan requirements while promoting high-quality, person-centered care.

Position Summary

The Lead Care Manager serves as the primary coordinator and leader of the ECM program, providing supervision, guidance, and support to Care Managers while ensuring members receive comprehensive, individualized care. This position collaborates closely with behavioral health providers, primary care providers, hospitals, community organizations, social service agencies, and other interdisciplinary partners to address the medical, behavioral, and social determinants of health affecting ECM members.

The successful candidate will help establish workflows, monitor program performance, maintain compliance, and contribute to the continued growth and success of the ECM program.

Essential Responsibilities

  • Provide leadership, mentorship, and daily support to ECM Care Managers.
  • Coordinate comprehensive care for members with complex medical, behavioral health, and social needs.
  • Conduct comprehensive needs assessments and develop individualized care plans.
  • Monitor member progress and ensure timely follow-up on care plan goals.
  • Coordinate services with primary care providers, behavioral health providers, specialists, hospitals, social service agencies, schools, and community partners.
  • Assist members in accessing housing resources, transportation, food assistance, employment services, and other community supports.
  • Ensure all documentation meets Partnership HealthPlan, CalAIM, Medi-Cal, and organizational standards.
  • Maintain accurate and timely documentation within the electronic health record.
  • Monitor productivity, quality measures, and program outcomes.
  • Participate in multidisciplinary case conferences and treatment planning meetings.
  • Identify barriers to care and develop creative, person-centered solutions.
  • Support quality improvement initiatives and program development.
  • Assist with staff onboarding, training, coaching, and performance improvement.
  • Ensure compliance with HIPAA, state regulations, and organizational policies.
  • Participate in community outreach and relationship-building activities.

Minimum Qualifications

Applicants should meet one or more of the following:

  • Bachelor's degree in Social Work, Psychology, Human Services, Nursing, Public Health, or a related field.
  • Master's degree preferred.
  • Licensed or registered behavioral health professional (LCSW, LMFT, LPCC, RN, or equivalent) preferred but not required.
  • Minimum of two years of experience providing care coordination, case management, behavioral health services, or community-based services.
  • Experience working with Medi-Cal populations.
  • Knowledge of social determinants of health and community resource navigation.
