Job description

Field Case Manager

Paradigm is an accountable specialty care management organization focused on improving the lives of people with complex injuries and diagnoses. The company has been a pioneer in value-based care since 1991 and has an exceptional track record of generating the very best outcomes for patients, payers, and providers. Deep clinical expertise is the foundation for every part of Paradigm's business: risk-based clinical solutions, case management, specialty networks, home health, shared decision support, and payment integrity programs.

Discover a different side of nursing at Paradigm. We specialize in complex and catastrophic workplace injuries and have been a pioneer in value-based care for over 30 years. In this role, you are an advocate for the injured worker / patient to ensure they get the right care at the right time helping them return to work or to the best quality of life as soon as possible.

As a Field Case Manager, you have the autonomy to manage your schedule. Below are some items to note:

  • The role requires travel within approximately a 2-hour radius from your home
  • It' not unusual for 60-70% of your day to be spent in the car or at doctors' offices, hospitals, rehab facilities, etc. The remainder of the time you work from your home office.
  • Your schedule will vary from day to day. Some days might start earlier while other days may end late.
  • Your time is flexible, but you must manage your time to ensure your appointments are attended and your reports are completed.

We want to set you up for success—this isn't just a saying, it's a guiding principle. As a new nurse to workers' compensation case management, Paradigm has a dedicated Training Department that offers a new hire onboarding program. For your onboarding and orientation period, you are assigned an onboarding manager who is your guide and mentor. The onboarding manager will meet with you regularly and be your go-to person when you have questions. During your orientation you will gradually build your case load as you shadow a current Case Manager. Again, we want you to succeed.

At Paradigm, you will experience meaningful work, exceptional people, and a collaborative culture. This role requires an RN license, the ability to operate autonomously, strong patient education skills, a valid driver's license and good driving history.

Paradigm believes that fostering a diverse and inclusive workplace is central to our mission of helping more people and transforming lives. We're striving to build a culture that better reflects the society we live in and empowers our team to deliver the highest levels of compassion and care to those we serve. For us, achieving this goal requires a workforce that respectfully embraces differences and commits to positive change, creating an environment where everyone is able to bring their whole self to work.

We are an Equal Opportunity Employer and do not discriminate against any employee or applicant for employment because of race, color, sex, age, national origin, religion, sexual orientation, gender identity, status as a veteran, and basis of disability or any other federal, state or local protected class.
