Position Overview

Bright Star Counseling is seeking a dedicated and compassionate Licensed Master Social Worker (LMSW) to join our team in delivering high-quality support to seniors. This full-time remote position provides the unique opportunity to make a significant impact in the lives of older adults and their families, ensuring their emotional, social, and overall well-being.

This position operates primarily in a remote setting, allowing for flexibility and the opportunity to connect with clients from the comfort of their homes. Key Responsibilities
  • Conduct comprehensive assessments to determine the psychosocial needs of seniors, evaluating emotional, mental, and social factors that may impact their well-being.
  • Collaborate closely with clients and their families to develop and implement personalized care plans that reflect individual goals and preferences.
  • Provide ongoing counseling and emotional support, facilitating the navigation of challenges related to aging, health transitions, and family dynamics.
  • Advocate for clients’ rights and ensure they have access to community resources and services that can enhance their quality of life.
  • Engage in interdisciplinary teamwork with nurses, caregivers, and other professionals to coordinate comprehensive care tailored to client needs.
  • Conduct regular visits and virtual check-ins to monitor clients’ well-being, assess living conditions, and address any concerns, including safety issues.
  • Foster communication among family members and facilitate resolution of conflicts, enhancing collaborative care approaches.
  • Educate clients and their families about supportive community resources, social engagement opportunities, and relevant programs.
  • Maintain meticulous documentation of client assessments, care plans, progress, and interactions in accordance with ethical standards.
  • Continuously research and stay updated on social work practices and policies to ensure compliance with best practices and regulatory requirements.
Patient Population / Client Focus

The primary clients served in this role are seniors who may be experiencing a range of challenges, including mental health issues, physical health transitions, social isolation, and complex family dynamics. As a Licensed Master Social Worker, you will be in a pivotal position to advocate for their needs and assist them in accessing critical resources that promote their independence and dignity in their later years.

Qualifications
  • A Master's degree in Social Work from an accredited institution is required.
  • Current and valid licensure as a Licensed Master Social Worker (LMSW) in Texas.
  • Demonstrated knowledge of social work principles, specifically as they relate to elderly populations and community resource navigation.
  • Strong assessment, counseling, problem-solving, and advocacy skills.
  • Ability to establish meaningful relationships with clients, demonstrating empathy and compassion.
  • Exceptional communication and interpersonal skills to collaborate effectively with interdisciplinary team members.
  • Proficient in using electronic health record (EHR) systems and other relevant technology.
Preferred Experience
  • A minimum of two years of experience in social work, particularly within a community center, healthcare, or geriatric setting is preferred.
  • Prior experience with electronic health records management and data tracking systems.
  • Familiarity with aging-related issues and community resources available for seniors.
