Job description

Help Bridge Hospital Care to Community Recovery
$5,000 Sign-On Bonus

Endeavor Health Services is seeking a Licensed Practical Nurse (LPN) to join our Critical Time Intervention (CTI) team. In this role, you will support individuals during key life transitions from institutional settings (such as hospitals or inpatient care) back into the community.

You will play a critical role in ensuring continuity of care, stability, and recovery, using a structured, time-limited intervention model focused on engagement, coordination, and long-term support connection.

What You’ll Do

  • Provide intensive case management and nursing support during transitions from institutional to community-based care
  • Conduct health assessments, monitoring, and vital signs (including weight, blood pressure, and other clinical indicators) within scope of practice
  • Develop and implement individualized care plans in collaboration with clients and interdisciplinary teams
  • Coordinate access to medical care, behavioral health services, housing, and community resources
  • Monitor client progress and adjust care plans based on evolving needs
  • Support hospital discharge planning and post-discharge stabilization
  • Educate the people we serve and families on health management, medications, coping strategies, and available supports
  • Advocate for the people we serve within healthcare systems and community service networks
  • Maintain accurate and timely documentation of all assessments, interventions, and care coordination
  • Participate in team meetings and case conferences to ensure coordinated, wraparound care
  • Collaborate closely with Population Health staff to support individuals with complex needs
  • Build and maintain relationships with community providers and referral partners

Compensation

  • Starting salary: $23.07 – $27.67/hour (based on experience and credentials)
