Position Overview

Breakthrough Rehabilitation & Treatment Center is actively seeking a dedicated and experienced Physical Therapist to join our team in Freehold, New Jersey. This full-time position offers a competitive salary ranging from USD 100,000 to 120,000 annually, along with a comprehensive benefits package. The ideal candidate will be a licensed Physical Therapist with a minimum of two years of experience in an outpatient orthopedic setting, ready to deliver high-quality patient care in a supportive clinical environment.

Work Environment

The position is based onsite at our outpatient office in Freehold, New Jersey, where you'll work in a spacious facility designed for efficient patient care. Our team is committed to maintaining a positive and empowering culture, making it a great place for personal and professional growth. Working hours are Monday through Friday from 8:00 am to 5:00 pm, allowing for a balanced work-life schedule.

Key Responsibilities
  • Conduct comprehensive patient evaluations to assess physical therapy needs, develop individualized treatment plans, and set achievable goals for recovery.
  • Implement evidence-based treatment protocols tailored to each patient’s unique condition and progress while continuously adapting strategies in response to ongoing assessments.
  • Maintain accurate documentation of patient care activities, including evaluation results, treatment interventions, and progress notes, to ensure compliance with legal and regulatory requirements.
  • Collaborate closely with a multi-disciplinary team including physicians, occupational therapists, and other health care professionals to discuss patient progress and adjust care plans as necessary.
  • Participate in weekly facility meetings to contribute ideas for overcoming barriers to patient access and improving overall service delivery metrics.
  • Complete discharge planning for patients and ensure smooth transitions to follow-up care and community resources.
  • Stay abreast of advancements in physical therapy by actively participating in continuing education opportunities and attending relevant training.
Patient Population / Client Focus

Our center focuses on treating patients with a variety of orthopedic conditions, ranging from post-operative rehabilitation to chronic pain management. You will work closely with individuals of all ages and backgrounds, making a significant impact on their recovery journey. Qualifications
  • A Doctorate of Physical Therapy (DPT) from an accredited program is required.
