Work Environment

This is a full-time remote position, allowing you to maintain a work-life balance while being part of a dynamic team. Our operational hours are Monday through Friday from 9 AM to 5 PM, providing the structure needed for efficient collaboration and service delivery. We use modern telehealth technology to facilitate seamless interactions with clients and other healthcare professionals.

Key Responsibilities
  • Conduct comprehensive psychosocial evaluations by engaging clients and their families to gather relevant information, which informs individualized treatment plans.
  • Monitor client progress by documenting achievements and setbacks in relation to the established care plan objectives, using both quantitative and qualitative measures.
  • Identify high-risk individuals and collaborate with your team to devise and implement effective prevention strategies, ensuring timely intervention when needed.
  • Work closely with other healthcare professionals to build integrated care pathways that enhance overall health outcomes and address psychosocial needs.
  • Regularly review and adapt treatment plans based on emerging client needs, cultural considerations, and feedback from the clients, families, and care teams.
Patient Population / Client Focus

This position serves a diverse client population within our community. We work with individuals experiencing a range of mental health challenges, including but not limited to anxiety disorders, depression, PTSD, and substance use issues. As a Clinical Social Worker, your insights and therapeutic interventions will be essential in empowering clients to achieve their mental health goals.

Qualifications
  • Master's degree in Social Work from an accredited program.
  • Current, active Licensed Clinical Social Worker (LCSW) license in the state of Oregon.
  • Proven experience in clinical evaluations and treatment planning, showcasing your ability to work with a variety of mental health issues.
  • Excellent collaborative and communication skills, necessary for effective teamwork and client relationships.
Preferred Experience
