Position Summary

Blueprint Healthcare is seeking a compassionate and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to provide part-time primary care services to residents in assisted living facilities.

This role emphasizes proactive, preventive, and longitudinal care, aimed at reducing hospitalizations, managing chronic conditions, and improving overall resident health and quality of life.

The ideal candidate is comfortable working independently in a mobile care setting and is passionate about geriatric and value-based care.


Key ResponsibilitiesPrimary Care Delivery
  • Provide comprehensive on-site primary care to residents in assisted living facilities
  • Conduct routine visits, assessments, and annual wellness exams
  • Evaluate and treat acute and chronic medical conditions
  • Develop, implement, and monitor individualized care plans
  • Order and interpret labs, diagnostic tests, and imaging
Preventive & Value-Based Care
  • Focus on early detection and intervention to prevent complications
  • Reduce unnecessary hospitalizations and ER visits
  • Perform medication reconciliation and optimize medication management
  • Identify high-risk residents and implement targeted care strategies
Collaboration & Care Coordination
  • Partner with facility staff, nurses, and caregivers to ensure continuity of care
  • Communicate clearly with residents and families regarding care plans
  • Coordinate with specialists, hospitals, and other providers
  • Work under the supervision or collaboration of a physician as required
Documentation & Compliance
  • Maintain accurate and timely documentation in EMR system
  • Ensure compliance with state, federal, and regulatory standards
  • Follow Blueprint Healthcare clinical protocols and guidelines

QualificationsEducation & Licensure
  • Graduate of an accredited Nurse Practitioner or Physician Assistant program
  • Current, unrestricted state license
  • Board certification (AANP, ANCC, or NCCPA)
  • Active DEA license (or ability to obtain)
Experience
  • Preferred: Experience in geriatrics, primary care, assisted living, or SNF settings
  • Strong clinical decision-making and diagnostic skills
  • Experience managing chronic diseases in older adults
