About the Role:

The Hospice Admissions Registered Nurse (RN) plays a critical role in providing compassionate and comprehensive care to patients and their families during the transition to hospice services. This position is responsible for conducting thorough patient assessments to determine eligibility and appropriateness for hospice care, ensuring that patients receive timely and personalized support. The RN collaborates closely with interdisciplinary teams, including physicians, social workers, and chaplains, to develop and implement individualized care plans that honor the patient’s wishes and clinical needs. Effective communication and empathy are essential as the RN educates patients and families about hospice philosophy, services, and what to expect throughout the care journey. Ultimately, this role ensures a smooth admission process that upholds the dignity and comfort of patients at the end of life while supporting their families emotionally and practically.

Minimum Qualifications:

  • Current and valid Registered Nurse (RN) license in the state of practice.
  • Minimum of 2 years clinical nursing experience, preferably in hospice, palliative care, or related fields.
  • Strong knowledge of hospice care principles, patient assessment, and symptom management.
  • Excellent communication and interpersonal skills to interact effectively with patients, families, and healthcare teams.
