Physical Therapist (PT) - Springfield, MN

Blue Stone Therapy is seeking a Physical Therapist (PT) to provide rehabilitation services to adult and geriatric patients at St. John's Circle of Care in Springfield, MN.


This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Physical Therapist is responsible for patient assessments, care plan development, and implementation of treatment programs designed to improve gait, balance, mobility, strength, range of motion, pain management, and overall functional independence.

This position will serve multiple partner communities, including Divine Providence Community Home and Prairie View Senior Living, with travel between locations as needed.


What You'll Do

  • Perform physical therapy evaluations and develop individualized plans of care
  • Provide skilled physical therapy treatment focused on functional and measurable outcomes
  • Address gait training, balance, mobility, strength, range of motion, pain, and discharge planning
  • Supervise Physical Therapy Assistants in accordance with licensure and regulatory standards
  • Maintain appropriate patient schedules per established guidelines
  • Complete timely, accurate documentation to support quality care and reimbursement requirements
  • Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes

Requirements

What You Bring

  • Graduate of an accredited Physical Therapy program
  • Current Physical Therapist license (or eligibility for licensure) in applicable state
  • Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred
  • Strong clinical reasoning, communication, and organizational skills
  • Commitment to patient-centered care and professional standards
