Job description

Location: Detroit Metro Area, Michigan
Work Location: Field-based patient visits
Employment Type: Part-time, PRN, and contract opportunities
Schedule: Flexible scheduling; weekday and weekend availability welcomed

Compensation

  • New-to-home-care LPNs: $35–$40 per visit during orientation
  • Eligible increase to $45 per visit after 90 days
  • Eligible increase up to $50 per visit after 6 months based on competency, reliability, documentation quality, performance, and available assignments
  • Experienced home care LPNs may be considered for a starting rate of $50 per visit based on verified experience and competency

Job Description

Bloom Care Staffing LLC is seeking dependable, compassionate, and professional Licensed Practical Nurses to provide high-quality care to patients in their homes throughout the Detroit Metro area.

This opportunity is ideal for both newly licensed LPNs who want structured home care training and experienced home health nurses looking for flexible per-visit work. Bloom Care Staffing values strong documentation, patient safety, professionalism, reliability, and respectful communication with patients, families, physicians, and agency partners.


Responsibilities

  • Complete scheduled skilled nursing visits in the patient’s home.
  • Perform focused nursing assessments and report changes in condition promptly.
  • Provide medication education and reinforce medication compliance.
  • Complete wound care, diabetic teaching, cardiopulmonary teaching, ostomy care, catheter care, PEG-tube care, and other assigned skilled services within the nurse’s scope and competency.
  • Reinforce fall prevention, infection prevention, diet, hydration, home safety, and emergency-plan education.
  • Document each visit accurately, completely, and on time.
  • Communicate with the supervising RN, physician, caregiver, and agency as required.
  • Follow the patient’s plan of care, physician orders, Bloom Care Staffing policies, and all applicable professional standards.
  • Maintain patient confidentiality and demonstrate respectful, dependable conduct.


Minimum Qualifications

  • Current, unrestricted Michigan LPN license
  • Current CPR or BLS certification
  • Valid driver’s license
  • Reliable transportation
  • Active automobile insurance
  • Ability to travel to patient homes throughout assigned service areas
  • Smartphone access for scheduling, communication, and documentation
  • Strong time-management, communication, and documentation skills
  • Ability to complete required onboarding, background checks, competency validation, and orientation
  • Home health experience preferred but not required


Orientation and Training

Orientation may include:

  • Supervised home visits
  • Chart review
  • Documentation training
  • Wound-care competency validation
  • Start-of-care observation
  • Recertification observation
  • Required nursing skills checkoffs


New graduates and LPNs who are new to home care are encouraged to apply
