Summary:
POSITION SUMMARY:
Primary responsibilities is interviewing parents to obtain information for the registration of birth with the state. Must be able to perform all duties required of a Specialist I including pick up, prepping and scanning of medical records.
POSITION EDUCATION/ QUALIFICATIONS:
• High school graduate /GED certificate or equivalent is required.
• Requires excellent customer service skills.
• Graduate of Certificate or Associates degree program in Health Information Management preferred.
• Computer skills preferred
• Good written and verbal communication skills required.
• Bilingual - English/Spanish required.
• Texas Driver's License under clear status with the DMV required.
JOB KNOWLEDGE/EXPERIENCE:
• Birth Certificate experience preferred.
• Medical Records experience in a hospital setting preferred.
• Working knowledge of computers systems required.
• Knowledge of the uses and limitations of medical records preferred
Responsibilities:
POSITION RESPONSIBILITES:
• Interviews each birth mother to obtain pertinent information to register the birth certificate in a customer friendly manner.
• Accurately abstracts the medical aspects of the birth registration via paper chart and Cerner (EHR).
• Accurately enters the information into the Texas Electronic Register
• Able to determine if an AOP is required. Understands the difference between a two party AOP, three part AOP, partial AOP and projected AOP. Is knowledgeable with the incarcerated parent program and absent parent instructions.
• Maintains Certification for AOP and Birth Registry Certification (BRC). Completes a minimum of 8 hours of approved educational courses for BRC re-certification.
• Accurately explains the AOP program to the parents.
• Reviews with and attains parental signature on the Verification of Birth Facts.
• Transmit the birth records to Vital Statistics via TER within the five (5) day filing deadline.
• Faxes AOPs to the appropriate entity in a timely manner including confirming received.
• Explains and receives authorization or denial from the parent for newborn registration with the Immtrac System.
• Updates discharged newborn's names in registration (Paragon).
• Maintains monthly Newborn Census Log and distributes weekly to appropriate designated personnel.
• Distributes daily birth totals to appropriate designated personnel.
• Must comply with all Policies and Procedures including confidentiality of Patient Health Information
• Performs other duties as assigned.
• • TRANSCRIPTION
• Monitor DocQroute System on an on-going basis for processing of rejected reports.
• Download recorded for physicians upon request.
• Assist physicians, Director, and HIM Analysis team with corrections to reports or locating of misplaced reports as needed.
• Compile Operative Reports via the use of approved templates for final approval and authentication by the physician.
• Assisting physicians with questions in regards to their Cerner "in-box"
• Re-creating of Power notes for corrections upon physician request and approval.
• Processing clinic visit notes for non-Cerner integrated physician offices.
• Creating volume by dictator reports- monthly or as requested.
• Monitor the status of the dictation system multiple times per day to ensure that all ports are up and working and the system is functioning properly. Report any abnormalities immediately.
• Monitor the dictation system on an on-going basis and make changes and updates as necessary to ensure optimal turn- around time.
• Communicate and educate the medical staff on an on-going basis. Notify any physician when there is an opportunity for improvement. Examples include but are not limited to:
• Carrying around a recorder for hours or days without docking it, causing a delay in transcription.
• Review all dictated jobs for accuracy as well as reality. If the type of dictation does not match physician, review the dictation
• INVOICES
• Compile invoices for contracted vendors such as: Physician Assistants, Creative, TSR, and MModal by verifying the invoices are billed correctly prior to Director's approval.

• B. Working Conditions: The individual spends over 95% of his/her time in an air-conditioned environment with varying exposures to noise. There is protection from weather conditions but not necessarily from temperature changes. The position does have low exposure to malodorous, infectious body fluids from patients and some minimal exposure to noxious smells from cleansing agents.
C. Occupational Exposure: This position has minimal to no exposure to blood, body fluids, or tissues and is an OSHA Category III (although situations can be imagined or hypothesized under which anyone, anywhere, might encounter potential exposure to body fluids). Person who performs these duties are not called upon as part of their employment to perform or assist in emergency medical care or first aid or to be potentially exposed in some other way.
