Who are we?

San Francisco AIDS Foundation (SFAF) envisions a future where health justice is achieved for all people living with or at risk for HIV. Our mission is to promote health, wellness, and social justice for communities most impacted by HIV through sexual health and substance use services, advocacy, and partnership. We need passionate advocates for social change that embody our five key values: Justice, Dignity, Courage, Leadership, and Excellence.

What is the position?

Health Navigators are key members of the SFAF team, responsible for facilitating seamless access to HIV prevention, care, and supportive services across both our agency and external partners. As a welcoming first point of contact at Strut our Castro sexual health clinic, Health Navigators ensure a warm, respectful, and safe environment while helping clients understand and access available services. The role supports the flow of daily operations, supports benefits and entitlements access, and maintains clean well-organized shared spaces.

What are the main responsibilities?

  • Greet and engage clients and visitors in person and by phone. Assess needs, provide accurate information and referrals, and guide clients to appropriate internal programs or external resources.
  • Register clients and check them into appointments as needed.
  • Assist clients in accessing PrEP, nPEP, HIV primary care, and sexual health services by coordinating benefits (e.g., Medi-Cal, Covered CA, ADAP, PrEP-AP), providing warm referrals, and supporting medication access relevant to SFAF's scope of care.
  • Maintain a clean, welcoming, and safe front-of-house environment at Strut.
  • Provide de-escalation and crisis intervention support when needed, ensuring a trauma-informed and culturally competent approach.
  • Work closely with the nursing, clinical, housing and financial benefits, and other Programs teams for referrals and linkage, coordinate follow-up, and co-manage care plans that promote client retention and engagement in services.

Who are we looking for?

  • High school diploma or equivalent and 4 years of relevant experience in community health, sexual healt education, benefits navigation, and customer service.
  • Experience working with our priority populations such as Queer, Trans, Gender Non-Conforming, and BIPOC communities.
  • Excellent interpersonal and intercultural communication skills.
  • Ability to manage multiple tasks with competing priorities in a fast-paced client facing environment.
  • Experience with data entry, service documentation, appointment scheduling, and administrative support.
  • Strong follow-through, problem-solving skills, and a high degree of reliability and professionalism.
  • Bilingual in Spanish and English.


Where is the position located?

This position is located at 470 Castro st, San Francisco, and will occasionally require traveling to other office locations or off-site meetings as required by the job. Role may include evening and weekend coverage.


What we offer?

Hourly Rate : $29.87 per hour.

Benefits : SFAF is proud to offer a comprehensive and competitive benefits package for our full-time employees. We offer 27+ days of combined Vacation/Sick Time Off for the first two years of employment, 32+ days of Vacation/Sick Time Off after two years of employment. For retirement we offer a 403(b) Plan and SFAF matches employee contributes (1 to 1) up to 3% of Salary.
