Job description

Job Overview: Required Master of Divinity (MDiv), Master of Theology, or equivalent degree from an Association of Theological Schools (ATS)-accredited institution. Completion of at least three (3) units of Clinical Pastoral Education (CPE) through an ACPE-, APC-, NACC-, or NAJC-accredited program. Three (3) to four (4) years of professional chaplaincy experience, including hospice, healthcare, or related ministry settings.

Demonstrated ability to assess and address spiritual and emotional distress in patients and families facing serious illness and end-of-life concerns. Preferred Four (4) units of Clinical Pastoral Education (CPE). Board Certification through one of the following organizations: Association of Professional Chaplains (APC) National Association of Catholic Chaplains (NACC) National Association of Jewish Chaplains (NAJC) Association for Clinical Pastoral Education (ACPE) Candidates who are not board certified must demonstrate a commitment to pursuing certification and meet with a certifying committee within three (3) years of hire.

Two (2) to three (3) years of chaplaincy experience beyond CPE training. Job Responsibilities Support chaplain services in times of census growth, vacation and LOAs, as directed by the Manager of Psychosocial and Spiritual Services. Provide spiritual assessments, interventions, counseling, rituals, and supportive presence based on patient and family needs, beliefs, and goals of care.

Develop and maintain individualized plans of spiritual care consistent with hospice philosophy and regulatory requirements. Document patient interactions and care plans accurately and timely in the electronic medical record. Respond effectively to spiritual, emotional, existential, and grief-related crises.

Provide support during times of decline, active dying, death, and bereavement. Assist patients and families in identifying sources of meaning, hope, reconciliation, and spiritual comfort. Participate actively in interdisciplinary team meetings and care planning discussions.

Collaborate with physicians, nurses, social workers, aides, volunteers, and other team members to ensure holistic patient care. Support colleagues and contribute positively to team functioning and organizational culture. Respond to referrals and requests for chaplain services in a timely manner.

Demonstrate flexibility, initiative, and effective time management in prioritizing patient care needs. Maintain professional standards, continuing education, and competency development consistent with organizational and certification requirements. Working Conditions Travel to patient homes and care settings within the assigned service area.

Exposure to emotionally intense situations involving serious illness, death, grief, and family crisis. Other Related Information Functions as a self-managed and collaborative member of the interdisciplinary team. Provides ministry in crisis situations and responds to urgent spiritual care needs.
