(Only QUALIFIED Healthcare Professionals accepted) Hospital/Medical Social Worker - A non-profit Federally Qualified Health Center serving adults, children and families on the Cape that offers a team-based model of care with a state-of- the-art facility and the Epic practice management system is seeking an experience Licensed Independent Clinical Social Worker (LICSW) Clinician to join their growing patient care teams.
Care at the patient-centered medical home is provided by a professional, supportive team of medical, behavioral health, dental, pharmacy and vision staff. Recognized as a Level 3 Patient Centered Medical Home, the centers vision is to be a health care home of choice reflecting community needs, empowering individuals toward lifelong wellness.
Responsibilities include providing culturally competent assessments, consultation, counseling and|or brief intervention to patients with psychological disorders. This is primarily for an adult population, though this can include some children and adolescents when appropriate.
Evaluating crisis situations and applying appropriate interventions and disposition of care plans. Assisting the primary care team to develop care-management processes such as behavior modification and disease management techniques, case-management and patient education to promote patients chronic disease self-management.
Providing an array of clinical services for patients with behavioral health challenges and areas of behavioral medicine (treatment planning|provision) to assist them in meeting their goals.
Requirements LCSW, LMHC required - LICSW preferred. Experience with group counseling and brief intervention modalities a strong plus.
