Role Overview
As a Home Health LPN/LVN, you provide direct patient care under the supervision of a Registered Nurse, helping to implement care plans and ensure patients receive safe, effective treatment in their homes.
You are a key part of the care team - delivering treatments, monitoring patient conditions, and reinforcing education to support successful outcomes.
What You'll Do
  • Provide direct patient care in accordance with the state Nurse Practice Act
  • Implement and support the plan of care developed by the RN
  • Deliver skilled nursing services, including treatments, medication support, and wound care
  • Monitor patient condition and report changes to the RN and care team
  • Educate patients and caregivers on self-care, treatments, and disease management
  • Maintain infection control and proper use of equipment and supplies
  • Document patient care accurately and timely within the EMR
  • Communicate effectively with physicians, nurses, and interdisciplinary team members
  • Participate in case conferences, team meetings, and ongoing training
  • Support coordinated care by maintaining consistent communication across the care team

What You Bring
  • Active LPN/LVN license in the state of practice
  • Graduate of an accredited practical/vocational nursing program
  • Current CPR certification
  • Minimum one (1) year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus)
  • Strong clinical and observational skills
  • Clear communication and patient education abilities
  • Ability to follow care plans while recognizing and reporting changes in condition
  • Basic proficiency with EMR systems
  • Valid driver's license and reliable transportation
