East Liberty Family Health Care Center (ELFHCC) is a mission‑driven Federally Qualified Health Center committed to providing whole‑person, high‑quality health care to the communities we serve. We are seeking a compassionate, community‑focused Community Health Worker (CHW) to support patients in accessing care, addressing social determinants of health, and improving overall wellbeing.Position SummaryThe Community Health Worker plays a vital role in connecting patients to resources, providing education, and building trusting relationships that support healthier outcomes.
