They typically work in hospitals, residential treatment centers, or outpatient facilities, helping patients with daily activities, monitoring their well-being, and assisting with therapeutic interventions under the supervision of licensed clinicians. As a Service Associate in our Life Services team in Black River Falls, you’ll play a key role in empowering individuals with disabilities to live more independent, self-directed lives. You’ll provide personalized, hands-on support across a variety of settings — including day services, supported living, and youth services. Your work will help participants build skills, explore their communities, and achieve meaningful personal goals.

RESPONSIBILITIES
  • Deliver person-centered support by facilitating group and one-on-one programming tailored to each participant’s goals (e.g., daily living, social skills, recreation, communication).

  • Assist with personal care tasks as needed, including hygiene, mobility, meal support, and medication reminders.

  • Support community inclusion by helping individuals engage in meaningful community activities, develop friendships, and build natural supports.

  • Advocate for participants by encouraging self-expression, exploring opportunities, and maintaining professional, respectful relationships with families, care teams, and community partners.

  • Complete accurate documentation including daily progress notes, service records, and time tracking.

  • Monitor health and safety, reporting any changes in behavior, health, or emotional well-being, and assisting participants with following care plans.

  • Provide safe transportation for participants, promote pedestrian safety, and support learning of alternative transportation methods.

  • Maintain professionalism and uphold ethical standards, attending required team meetings and training.

  • Follow compliance and safety procedures aligned with local, state, and federal regulations, and Aptiv's internal policies.

QUALIFICATIONS
