Job description

Summary

The Social Worker 2 provides advanced skilled interventions for the support and/or resolution of patient and family crises, problem solving and decision-making advocacy, facilitation necessitated by life-changing events, mediation of risk factors, coping with disease or disability, and other psychosocial and/or socioeconomic issues. Is a resource for the Social Worker 1.

Education

Masters Degree in Social Work or related field required.

Certification, Registration & Licensure

Experience

Minimum of three years clinical Social Work in a hospital, Skilled Nursing facility, Home Health or Hospice setting. Demonstrates effective interpersonal, organizational and motivational skills. Demonstrates ability to function as a team player. Proficient in written and verbal communication.

Responsibilities

  • Upon consultation, conducts a comprehensive assessment of the patient's behavioral, psychological and

    socio-economic needs.

  • Attends Care Planning or Discharge Planning rounds to facilitate communication and the importance of

    the patient and family's goals.

  • Participates in end of life discussions and advance care planning.

  • Bases social work actions by code of ethics according to National Association of Social Workers (NASW).

  • Documents measurement and evaluation of social service consultation and satisfaction outcomes as

    patient progresses towards desired goals.

  • Provides leadership to departmental staff.

  • Performs other duties as assigned.
