Job description

As the Resident Advocacy Director (RAD) at the Legacy of Lexington, you will be a key partner in resident care - leading clinical coordination, care management, behavioral health support, and the Navion Cares implementation to ensure resident wellbeing and health.
You will lead enrollment in Medicare care coordination programs, develop personalized care strategies including behavioral health services, and drive collaboration across interdisciplinary teams and external partners.

Compensation: $75,000+ per year depending on experience, with potential for 15% annual bonus
Requirements
  • Active Licensed Independent Social Worker - Clinical Practice (LISW-CP) or Licensed Professional Counselor (LPC) credentials in South Carolina
  • Strong clinical background with proven experience in care management or a direct care role within a healthcare setting
  • Experience delivering behavioral health interventions and psychotherapy
  • Experience in geriatric care, particularly with dementia and cognitive decline
  • Working knowledge of clinical workflows, resident assessments, and care plan development
  • Comfort with electronic health record (EHR) systems
  • Highly organized and self-motivated, with the ability to manage a caseload of residents
  • Exceptional ability to build rapport and communicate with residents, families, and team members

Key Responsibilities
As the central care coordination specialist for residents at the Legacy of Lexington, you will shape how we deliver clinical support to residents and families.
  • Program Enrollment & Care Management: Enroll eligible residents in care coordination programs suited to their diagnoses and person-centered care plan goals, then fulfill all program requirements and monthly duties so each care plan delivers the right level of support to meet the resident's goals.
  • Behavioral Health & Psychotherapy: Support the emotional and psychological wellbeing of residents by facilitating psychosocial assessments, deliver individual and group psychotherapy as needed, and align behavioral health interventions with each resident's overall care plan.
  • Partnerships & Service Coordination: Serve as the key support person and provide care coordination for residents receiving medical, hospice, home health, therapy, and pharmacy services. Work alongside Navion's clinical team to manage referrals, scheduling, and documentation, and communicate regularly with residents' primary care providers so every service is seamless and resident-focused.
  • Resident, Family, & Team Engagement: Build trusted relationships with residents, families, and interdisciplinary teams through regular check-ins, clear communication, and proactive advocacy-ensuring preferences are honored and concerns are addressed. Play a key role in care plan meetings.
  • Community Team Partnership: Partner with the Director of Clinical Services, nursing team, and Resident Care Coordinators to take on key care coordination tasks and coach on workflows and documentation-freeing colleagues to focus on resident care needs and strengthening overall team performance.
