Our clients are supported by a team of mental health counselors, social workers, certified nurse practitioners, case managers and psychiatrists to ensure the best outcomes for those we serve. Responsibilities may include:
Managing a team of TBS Specialist by providing professional support, training and mentorship.
Managing a caseload of clients at 30% productivity.
Connecting clients with community resources.
Ensuring that the treatment/behavior plan is being followed.
Ensuring all required documentation is completed and sent to the appropriate supervisor.
Communicating and following up as needed regarding the client's status.
Working collaboratively with those providing therapy, counseling and/or psychiatric services.
Requirements for this position include:
Bachelor's degree in Social Work or a related field preferred
Experience in community-based case management.
Managerial experience is preferred.
Valid driver's license, car insurance and reliable transportation.
Willingness to travel throughout assigned service area. This position will require regular travel toLima, Rockford, Paulding and Hicksville.
