Job description

Baystate Health, a nationally recognized leader in healthcare quality and safety, and home to Baystate Noble Hospital is looking for an experienced Crisis Clinician to join our Emergency Department team in Westfield, MA!
SCHEDULE:
  • Part-time, 24 hours/week
  • 2nd shift, 8 hour shifts, flexible 2pm-12am, 1 weekend/month required
  • A/B rotating holiday schedule
  • Must be able to travel to support Baystate Medical Center ED (Springfield, MA) and Baystate Wing Hospital ED (Palmer, MA)
  • Position to transition to solely support Baystate Noble Hospital ED in the future.

LOCATION:
  • Baystate Noble Hospital- Westfield, MA
  • Baystate Medical Center- Springfield, MA
  • Baystate Wing Hospital- Palmer, MA

JOB SPECIFIC DUTIES/RESPONSIBILITIES
  • Obtains and maintains release of information forms, and gathers collateral information from outside resources including family, friends, providers, and other community resources
  • Completes a psychosocial evaluation including current and past history, assessment of mental status, and risk assessment through patient interviews, collateral contacts, and record review.
  • Monitors and assists in the completion of legal documentation (CV's, 3-Day Notices, Section12's, & Section 7/8 forms).
  • Enters data and reviews charts for quality assurance, program statistics, tracking of patient progress, and/or tracking of clinical outcomes as assigned.
  • Completes a discharge plan which addresses follow-up recommendations, step-down / placement options, transportation issues, or other treatment needs.
  • Other duties and responsibilities as required.

MINIMUM QUALIFICATIONS:
  • Master's degree required.
  • LICSW or license eligible required.
  • Experience in behavioral health setting required.
