Job description

Licensed Practical Nurse (LPN) – Home Health Visits

Full-Time | Hudson County, NJ

Immediate Full-Time Opportunity Available!

BAYADA Home Health Care is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Hudson County Visits team.

In this role, you'll provide one-on-one skilled nursing visits to adult clients throughout Hudson County, NJ, helping them safely remain in the place they call home. As a valued member of our interdisciplinary care team, you'll deliver high-quality clinical care while building meaningful relationships with your clients.

A minimum of one year of clinical experience as a Licensed Practical Nurse is required.

Responsibilities
  • Provide skilled nursing visits to clients throughout designated territories.
  • Administer medications, wound care, treatments, and other physician-ordered nursing interventions.
  • Monitor client conditions and communicate changes to the Clinical Manager and Client Services Manager.
  • Complete comprehensive nursing assessments as needed.
  • Execute and document physician orders accurately and in a timely manner.
  • Coordinate care with physicians and the interdisciplinary care team.
  • Maintain accurate documentation of client assessments, interventions, and outcomes using BAYADA's electronic medical record (EMR) system.
Qualifications
  • Current New Jersey Licensed Practical Nurse (LPN) license.
  • Minimum of one year of recent, verifiable clinical experience as an LPN.
  • Graduate of an accredited nursing program.
  • Home health experience strongly preferred, but not required.
  • Ability to work independently and manage time effectively.
  • Strong communication, organizational, and interpersonal skills.
  • Experience with electronic medical records (EMR) preferred.
  • Reliable transportation and ability to travel throughout the assigned territory.
