Rochester Hills, MI, 48307 - Onsite

Experience Level: 1-3 years
Department: Float Staffing
Shift Timings: 19:00- 07:30 Hours Monday to Friday 3 Days/12 Hours
Per Week: 36 Hours
Roles & Responsibilities
Implement and monitor patient care plans.
Monitor, record, and communicate patient condition as appropriate.
Serve as a primary coordinator of all disciplines for well-coordinated patient care.
Note and carry out physician and nursing orders.
Assess and coordinate patient's discharge planning needs with members of the healthcare team.
