Los Barrios Community Health Clinic seeks a dedicated and compassionate Licensed Independent Clinical Social Worker (LICSW) for a full-time remote position. In your role as a Licensed Independent Clinical Social Worker, you will:

• Conduct comprehensive psychosocial assessments to identify emotional, social, and financial needs of patients as they transition into hospice care.
• Provide both individual and group counseling sessions to patients and families, addressing issues such as grief, adjustment, and coping mechanisms related to serious illness.
• Coordinate with community organizations and resources to arrange post-discharge services, ensuring effective continuity of care for patients and families.
• Contribute to the development and implementation of discharge plans tailored to patients' needs, leading discussions with interdisciplinary teams to optimize patient outcomes.
• Participate in hospital committees, utilizing your expertise in social work to inform policies and practices that enhance patient care and support.
• Document patient interactions and care plans accurately and promptly, adhering to hospital policies and legal requirements.
• Implement crisis intervention strategies when needed, offering immediate support and counseling to patients facing acute emotional challenges.

This role primarily focuses on patients in a hospice center, many of whom are facing terminal illnesses. The emphasis is on providing emotional support and practical resources to patients and their families during a sensitive time. You will advocate for their needs, ensuring that all care is culturally competent and aligned with patients’ values and preferences.

To qualify for this position, you must possess:

• A Master’s degree in Social Work from an accredited institution.
• An active license as a Licensed Independent Clinical Social Worker (LICSW) in the state of Washington.
• Strong knowledge of mental health disorders and crisis intervention techniques, coupled with familiarity with hospice care dynamics.
• Excellent communication and interpersonal skills, capable of engaging effectively with patients, families, and healthcare teams.

While not mandatory, candidates with the following experience are encouraged to apply:

• Two years of experience in a healthcare or hospital setting, particularly within hospice or palliative care environments.
• A solid understanding of medical terminology as related to social work practice in healthcare settings.
