Provide support coordination & case management services to adults with developmental/intellectual disabilities. Assure that individuals receive appropriate services and assist them in accessing needed medical, psychiatric, educational, residential and other support services. Bachelor's degree in human services field preferred & one-year experience working with the population required. Experience with assessment & service plan development desirable. Must be computer literate. Salary is determined by education & experience as it relates to the position. Full time, with benefits.
