Job description

Job Summary: Clinical Supervision provided for CSAC-A and CSAC-R

EARLY MORNING HOURS 5:30am - 2:00pm
We are seeking a Care Coordinator to join our team and assist in coordinating care services for our clients. The Care Coordinator will be responsible for assessing client needs, developing care plans, coordinating with healthcare providers, and ensuring the delivery of high-quality care.
Responsibilities:
1. Conduct initial assessments to determine client needs and develop personalized care plans.
2. Coordinate care services and schedules for clients, including medical appointments, therapy sessions, and home care services.
3. Communicate effectively with clients, families, and healthcare providers to ensure continuity of care.
4. Monitor and evaluate client progress and adjust care plans as needed.
5. Document all client interactions and progress accurately and timely.
6. Provide guidance and support to clients and their families regarding available resources and services.
7. Collaborate with interdisciplinary teams to optimize client care outcomes.
Qualifications:
1. Bachelor's degree in healthcare management, social work, nursing, or related field.
3. Strong communication, organizational, and problem-solving skills.
5. Ability to work independently, prioritize tasks, and meet deadlines.
6. Compassionate attitude and ability to maintain confidentiality.
7. Proficiency in basic computer skills and electronic medical records systems.
