The position is primarily onsite and involves home visits to provide therapeutic services to our patients in the comfort of their homes. You will experience a dynamic work environment that allows for one-on-one interaction with patients, fostering strong therapeutic relationships. Working collaboratively with an interdisciplinary team, including physicians, case managers, and other healthcare professionals, will enhance your practice and provide holistic care to our patient population.

• Provide comprehensive care for both inpatient and outpatient cases, while maintaining effective collaboration with an interdisciplinary team to address each patient's unique needs.
• Deliver high-quality evaluations and treatments for assigned patients using standardized assessment tools and measures. Set realistic and achievable goals that align with each patient's rehabilitation plan.
• Maintain meticulously detailed documentation within our electronic medical record system for each evaluation and treatment session, ensuring accurate billing and compliance with legal standards.
• Perform screenings for appropriate patients, which may include assessments for lymphedema, fall risks, and rehabilitation needs.
• Design and implement specific, individualized treatment programs based on best practices and evidence-based therapy protocols.
• Utilize various therapeutic techniques to restore and enhance functional abilities, improve mobility, manage pain, and prevent the onset or progression of disabilities.
• Participate in opportunities to contribute to departmental education efforts, present at conferences, and supervise students or interns, which provides professional growth and advancement.

You will primarily serve patients in home environments who face a wide range of conditions, including but not limited to orthopedic injuries, neurological disorders, chronic pain syndromes, and developmental disabilities. Your expertise will be vital in creating customized rehabilitation strategies that respect each patient's background and specific challenges.

• Current and valid Florida Physical Therapy License.
• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• A minimum of one year of clinical experience in an acute care hospital setting.
• Expertise in trigger point therapy techniques and a strong understanding of human anatomy and physiology.
• Ability to work effectively with pediatric patients and adapt treatment methods accordingly.

• Familiarity with hospital medicine practices and inpatient care.
• Proficiency in providing therapy in both clinic and home environments.
• Experience working within an interdisciplinary team setting and adapting therapy practices as needed based on collaborative feedback.
