Job description

Job Duties

This position is responsible for providing case management and public health services to children and adults including health assessment and care plan development. Duties involve providing professional health care delivery and services in the areas of health promotion, disease prevention, environmental health and disaster preparedness.

Conducts various client health assessments, analyzes and interprets assessment results, plans, and/or provides case management services to at-risk clients, children or adults under various department programs, clinics, or screenings. Performs such duties and tasks as:

o Researches and implements “evidenced-based practices”;

o Consults with physicians and other health professionals and develops individual care plans;

o Provides skilled professional nursing services in accordance with nursing guidelines, orders, and

plans;

o Assesses the progress of clients against care plans and adjusts accordingly;

o Conducts assessments at Long-Term Care Consultation, MnChoice Assessments, MCH

Programming and managed care processes;

o Performs case management services for clients and coordinates services for clients under

Medicaid waivers or managed care organizations; authorizes and approves services provided

under these programs;

o Documents and completes all forms, observations, services, and reports required for local, state

or federal agencies.

o Coordination and implementation of Children and Youth with Special Health Needs (CYSHN)

program, this includes birth defects monitoring, follow along program coordination, Early Hearing

Detection and Intervention monitoring.

o Coordination of WIC peer counselor.

o Performance of Family Home Visiting using universal model as well as MECSH evidenced based

model.

o Monitoring of children with elevated lead levels and follow-up.

o Coordination and follow-up with Perinatal Hepatitis B clients.

 Assists in the assessment and identification of health promotion and disease prevention concerns and issues

within the County as part of the formulation of the Community Health Assessment and Community Health

Improvement Plan. Performs such duties and tasks as:

o Implements programs to disseminate health education information to groups and individuals in

the areas of parenting and emergency preparedness.

o Evaluates the effectiveness of current programs and makes recommendations on changes or new

programs.

o Develops partnerships and efforts with other community agencies to address identified health problems and concerns (e.g. H1N1 Covid-19).

o Monitors disease prevention and control data

o Coordination of the Child & Teen Check-Ups outreach grant

 Conducts public health nursing clinics, WIC clinics, and water testing services. Performs such duties and

tasks as:

o Interprets immunization schedules and administers vaccines;

o Conducts screenings for health risk appraisals including high blood pressure, cholesterol, and foot

care.

o Analyzes screenings, interprets results, and makes appropriate referrals.

o Makes referrals for physicians or medical treatment, provides health education on disease

prevention for inmates and staff, administers Mantoux testing;

o Conducts WIC screening activities and counsels high risk mothers and children.

o Performs water testing for private residents, City of International Falls and Voyagers National Park

for E.coli and Coliform.

 Researches, recommends and writes grant applications to obtain funding for new health care programs and

services. Determines grant objectives, plans and anticipated outcomes/benefits; monitors and tracks grant

expenditures; evaluates program effectiveness against stated program objectives through surveys or

outcome measures; prepares reports, summaries or documentation required by funding agencies.

 Assists in planning, developing, implementing and evaluating the public health emergency preparedness

plan for the County.

o Plans and implements mass dispensing clinics and the distribution of medical and pharmaceutical

supplies.

o Develops, plans and recommends department protocols to coordinate agency response.

o Recommends county-wide plan to coordinate communication of public health information to

providers, hospitals, law enforcement and/or private agencies.

 Performs other duties of a comparable level/type, as required.
