  • Location: 1920 Libal St, Green Bay, WI 54301
  • Hours: 32 hours/week
  • FTE Status: 0.80 FTE,Full Time
  • Work Schedule: Four, eight-hour days between Monday - Friday, no weekends or holidays

Job Description:
  • Responsible for providing case management/social work services in the home. Working on a team to assist with guiding our patients to have the best possible health outcomes by eliminating barriers to health care, helping with psychosocial needs, addressing financial concerns and connecting to resources in the community. Also assist with evaluating and eliminating social determinants of health barriers and assuring smooth care transitions throughout the continuum of care.
  • This position includes a mix of travelling to patient homes for in-person visits as well as virtually through telephone and video visits from the Home Health office.
Qualifications:
  • Master's degree in Social Work required.
  • Wisconsin Advanced Practice Social Worker license required.
  • Valid Wisconsin driver's license required.
  • One year of Social Work experience in a health care setting required.
