Key Responsibilities

  • Perform initial and ongoing assessments to develop and update personalized care plans
  • Deliver skilled nursing services and specialized procedures per physician orders
  • Complete timely, accurate documentation including OASIS assessments
  • Educate patients and families on safety, medications, and health conditions
  • Communicate with physicians regarding changes in patient condition
  • Supervise and support LPNs and HHAs, including conducting supervisory visits
  • Coordinate care with the interdisciplinary team and participate in team conferences
  • Ensure compliance with all clinical, regulatory, and documentation standards

Requirements

What We're Looking For

  • Active RN license in the state of New Mexico
  • At least 1 year of clinical nursing experience (home health preferred)
  • Proficiency in OASIS documentation and home health regulations
