Care Connect at Home – Boca Raton, FL

Care Connect at Home is a family-owned and operated home care agency seeking an experienced and dependable Registered Nurse (RN) to join our team on a PRN / as-needed basis.

This is an ideal opportunity for an RN looking for flexible supplemental work without a set weekly schedule. There are no guaranteed hours. The RN will be scheduled and compensated as nursing needs arise within the agency.

Responsibilities
  • Conduct initial and follow-up client assessments as needed

  • Develop and update client care plans

  • Perform supervisory visits and reassessments when required

  • Review client conditions and identify changes in care needs

  • Provide clinical guidance related to client care

  • Maintain accurate and timely documentation

  • Assist the agency with RN-related clinical and compliance requirements

  • Communicate professionally with clients, families, caregivers, and agency leadership

Qualifications
  • Active, unrestricted Florida Registered Nurse (RN) license

  • Home care, home health, senior care, or similar experience preferred

  • Strong assessment and documentation skills

  • Reliable transportation and ability to travel to clients when needed

  • Professional, dependable, responsive, and comfortable working independently
