What You'll Do:
  • Deliver personalized hospice care that honors each patient's unique needs-document every step to ensure accuracy and continuity.
  • Lead with empathy during home visits, assessing patient and family needs and crafting care strategies that prioritize comfort, dignity, and peace.

  • Maintain compliance and eligibility by monitoring patient status and completing all certification and recertification documentation with precision.
