Job description

Transitions At Home, is currently seeking to hire a compassionate and experienced Full-Time RN Case Manager to join our growing home health team. This position offers the opportunity to provide personalized care in a home setting, ensuring clients receive the highest quality of service and support throughout their recovery journey.

What does a Transitions at Home RN Case Manager do?
Transitions At Home Registered Nurse Case Managers provide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the client and family/significant others in their place of residence in accordance with the
patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
Hours: Full-Time
  • 40 hours per week 8:00 am - 4:30pm
  • Monday through Friday
  • Limited on-call rotation
