What is an at home RN?

At Home RNs, also known as home health registered nurses, are licensed nurses who provide medical care to patients in their own homes. They typically assist patients recovering from illness, surgery, or injury, and may also care for those with chronic health conditions. Their responsibilities include administering medications, monitoring vital signs, educating patients and their families, and coordinating with other healthcare professionals. At Home RNs help ensure patients receive quality care outside of a traditional hospital or clinic setting, promoting recovery and independence.

What are some common challenges faced by an at home RN, and how can they effectively manage them?

At Home Registered Nurses (RNs) often encounter challenges such as working independently without immediate access to colleagues, managing diverse patient needs in non-clinical environments, and adapting to varying home conditions. To effectively manage these challenges, At Home RNs should maintain strong communication with their healthcare team, utilize telehealth tools for support, and stay organized with thorough documentation. Building rapport with patients and their families is also crucial to ensure smooth care delivery and address any concerns promptly.

What are the key skills and qualifications needed to thrive as an at home RN, and why are they important?

To thrive as an At Home RN, you need strong clinical assessment skills, nursing credentials (such as an active RN license), and experience in home health care. Familiarity with telehealth platforms, remote patient monitoring systems, and electronic health records is typically required. Exceptional communication, independence, and time management are vital soft skills for working autonomously and supporting patients in their own environments. These competencies ensure safe, effective care delivery and foster trust while adapting to the unique challenges of home-based nursing.

Job description

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient's care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.
At Bluegrass Care Navigators we are committed to a work setting that treats all team members with fairness, dignity, and respect. We offer our team members an opportunity to grow, to develop professionally, and to work in a team environment. You can help our team make a lasting difference. Join #TeamBCN where your work matters, and your time is valued.
Schedule: PRN (Monday-Friday 8:00am-5:00pm)
Position Summary:
The PRN Hospice RN provides intermittent, as needed care to Hospice patients in a home care setting, or wherever the patient calls home. Home care settings may consist of a private residence, skilled nursing facility, personal care home, and/or other interim living arrangements. The PRN Hospice RN supports the core hospice team by filling in during staffing absences, periods of high census, or special patient needs. A Registered Nurse in this position will have strong communication skills, the competence to work autonomously, professionalism to work within an interdisciplinary team, and the ability to provide care with empathy. This position offers flexible scheduling and a meaningful opportunity to make a difference in patients' lives during a challenging time.
Requirements
โ€ข A graduate of an accredited school of registered nursing.
โ€ข Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or participating COMPACT state Board of Nursing.
โ€ข One year of general nursing experience is preferred. New nursing graduates will be considered.
โ€ข Skilled in nursing practice and able to maintain professionalism in moments of emotional stress.
โ€ข Provide care in a compassionate nature while serving a diverse population respectfully and professionally.
โ€ข Complies with accepted professional and ethical standards and principles.
โ€ข Understands and supports the hospice philosophy and principles of death and dying.
โ€ข Possesses and maintains current CPR certification.
โ€ข Have satisfactory references from nursing schools, previous (or current) employers and/or professional peers.
โ€ข Have excellent observation, good nursing judgment and communication skills. Ability to provide accurate and timely documentation.
โ€ข Self-directed to work with minimal supervision and within an interdisciplinary team
โ€ข Flexible and cooperative in fulfilling role obligation to meet the needs of patients and BCN.
โ€ข Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
โ€ข Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage.
Bluegrass Care Navigators is committed to creating a diverse environment and complies with Equal Employment Opportunity and Title VI guidelines. Employment with Bluegrass Care Navigators, as well as our services, are available to all qualified individuals. We embrace and celebrate differences of persons of varying age, ethnicity, gender, disability, race, color, sex (including sex characteristics, intersex traits, pregnancy or related conditions), sexual orientation, gender identity, sex stereotypes, religion, national origin, political affiliation, socioeconomic and family status, geographic region, and other characteristics that make our employees unique.
