About This Role

As the Registered Nurse in Home Health, you will provide and direct nursing care to patients in their homes as prescribed by the physician, ensuring compliance with applicable laws and regulations. This role is ideal for those who enjoy working independently and coordinating care with other healthcare professionals.

What You'll Do
  • Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other health care professionals.
  • Supervise care provided by home health aides and licensed practical/vocational nurses.
  • Provide required supervisory visits.
  • Provide high-quality clinical services within the scope of practice and infection control standards.
  • Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
  • Document patient visits per policy and payer requirements.
  • Make initial and/or comprehensive nursing evaluation visits to ensure home health eligibility.
  • Communicate relevant information timely and effectively with appropriate agency staff.
  • Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
  • Participate in on-call and weekend rotation as needed to meet patient needs.
RequirementsEducation:
  • Current and unrestricted RN licensure in state of practice
Experience:
  • 1+ years of Home Health experience preferred
Licenses & Certifications:
  • Current Driver's License required
  • Current CPR Certification preferred or ability to complete within 90 days of hire
