Summary
We are seeking to add caring, compassionate Physical Therapists to our growing team. Our Physical Therapists will deliver individualized, evidence-based therapy to support patient recovery and maximize mobility. They will evaluate patient needs, develop individualized treatment plans, and implement therapeutic activities tailored to each patient's needs and environment. This role involves close collaboration with patients, families, and interdisciplinary teams to ensure safe, effective, and goal-oriented care.
Key Responsibilities:
  • Assessment & Evaluation-Conduct comprehensive physical assessments, review medical history, and identify functional limitations to develop individualized care plans.
  • Treatment Planning & Intervention-Design and deliver evidence-based therapeutic exercises, manual therapy, modalities, and rehabilitation programs tailored to patient goals.
  • Patient Education & Advocacy-Equip patients and caregivers with instruction on home programs, injury prevention, adaptive strategies, and lifestyle modifications to support progress.
  • Interdisciplinary Collaboration & Documentation-Communicate with physicians, nurses, and other care providers while maintaining timely, accurate clinical documentation and progress reports.
  • Outcome Management & Quality Improvement-Monitor patient outcomes, adjust plans based on response, and participate in quality assurance activities to drive positive patient results and compliance with standards.
