Position Summary
Delta Health is seeking a compassionate and skilled Home Health Registered Nurse (RN) to join our team. This role maintains the highest standards of professional nursing practice in the home care setting under the supervision of the Home Health Administrator. The RN is responsible for assessing, planning, and delivering patient care within the scope of their training, licensure, and organizational policies, as well as state and federal guidelines.


Key Responsibilities

  • Provide nursing care through all phases of treatment, considering each patient's age and developmental needs.
  • Collaborate with physicians and the healthcare team to support and implement individualized care plans.
  • Conduct periodic patient re-evaluations and adjust care plans as needed.
  • Perform OASIS assessments and documentation; deliver treatments according to established protocols.
  • Complete systematic, ongoing assessments focusing on physical, psychological, cultural, cognitive, and spiritual needs.
  • Support patient and family education, provide counseling, and promote patient advocacy.
  • Facilitate timely and appropriate patient discharge based on goals and policies.
  • Supervise and educate LPNs and HHAs as required by law and practice standards.
  • Document all care, interventions, and coordination per legal and agency requirements.
  • Promptly notify physicians and relevant staff of significant patient changes.
  • Coordinate multidisciplinary care and consult with other health professionals as necessary.
  • Manage workload, set priorities, and complete tasks independently within established timeframes.

Required Education & Experience

  • Graduate of an accredited nursing school; BSN preferred.
  • Active Colorado Registered Nurse license.
  • Minimum 2 years of RN experience; Home Health experience strongly preferred.
  • Strong clinical assessment and critical thinking skills in both adult and pediatric populations.
  • BLS certification required.
  • All licensure and certifications must be obtained per current policy.
