HouseCalls Home Care is a mission-driven Licensed Home Care Services Agency (LHCSA) providing high-quality, compassionate care to elderly and disabled individuals throughout New York City. We collaborate with hospitals, physicians, and families to support patients’ independence through personalized care delivered at home.

Position Overview

We are seeking experienced and compassionate Registered Nurses (RNs) to perform in-home visits across all NYC boroughs. As a Fee-for-Service Registered Nurse, you will play a vital role in patient care, assessments, and care coordination—helping clients live safely and comfortably in their homes.

Compensation (Per Diem Rates)

  • Initial Assessment Visit: $130
  • Reassessment Visit: $100
  • Resumption of Care Visit: $100
  • Supervisory Visit: $80

Key Responsibilities

  • Conduct initial and follow-up in-home nursing assessments
  • Develop and update individualized care plans
  • Educate patients and families on disease management and self-care
  • Supervise and instruct caregivers and home health aides as needed
  • Complete timely and accurate documentation in compliance with regulations
  • Communicate effectively with interdisciplinary team members

Qualifications

  • Active New York State Registered Nurse (RN) license
  • 1–2 years of experience in CHHA or long-term care preferred
  • Strong clinical assessment and documentation skills
  • Professional liability insurance is required
