Job description

We are seeking a dedicated and compassionate Home Care Nurse to join our team. In this role, you will provide comprehensive nursing care to residents within our community, ensuring their health and well-being in the comfort of their own apartments. This is an exciting opportunity to work in a supportive environment where you can make a meaningful impact on the lives of our residents without the need for travel.

SUMMARY OF JOB:

The Registered Nurse is authorized to take any reasonable action necessary to carry out the responsibility delegated to her/him so long as such action does not deviate from established policy and practice as defined by Pennswood Village and the current Pennsylvania Professional Nursing Act in the management of the Home Care Supervisor within all levels of care.

  • Apply the nursing process to residents in the Health Center and to residents in their homes to provide for their health care needs.
  • Participate in assessing, planning, implementing and evaluating the individual patient plan of care as a part of the interdisciplinary team.
  • Maintain accurate and objective written and oral communication.
  • Carry out professional responsibilities both independently and in collaboration with other staff. Plan and manage patient care assignments.
  • Responsible for distribution of prescribed medications to residents.
  • Provide demonstration, supervision, and evaluation of care given by nurses aides and the orientation of new staff.
  • Participate in the development of the patient care policies and the standards of nursing practices.
  • Establish safe and accurate working habits in the delivery of individual patient care.
  • Consistently seek to extend professional knowledge and improve skills.
  • Maintain and ensure the personal rights of all patients through knowledge and leadership in advocating the Patient's Bill of Rights.
  • Actively participate in monitoring the medical alert and fire alarm systems.
  • Perform clinical laboratory procedures.
