Position Overview

Admissions RN Signature Healthcare at Home is seeking a fulltime, hourly Home Health Admissions RN to support our Portland Metropolitan service area. This role is ideal for nurses who excel in firstvisit assessments, careplan initiation, and establishing a strong foundation for each patient's homehealth episode.

This position follows standard business hours, Monday through Friday, providing predictable scheduling, strong interdisciplinary support, and a balanced workflow.

Key Responsibilities

Admissions RN Responsibilities

  • Complete comprehensive StartofCare (SOC) nursing assessments in patient homes.

  • Initiate individualized care plans and establish expectations for the homehealth episode.

  • Conduct medication reconciliation, safety assessments, and diseasespecific education.

  • Coordinate care with interdisciplinary team members to ensure timely service initiation.

  • Provide skilled nursing interventions as needed during SOC visits.

  • Maintain accurate, timely documentation in accordance with regulatory and organizational standards.

  • Communicate effectively with physicians, caregivers, and internal team members.

  • Support safe transitions from hospitals, SNFs, and outpatient settings.

Business Hours

  • Monday-Friday

  • Normal business hours (daytime schedule)

  • No evenings, nights, or rotating weekends

  • Predictable workflow with full team availability during working hours

Qualifications

RN Requirements

  • Current Oregon RN license (or ability to obtain).

  • One year of nursing experience preferred; homehealth or admissions experience a plus.

  • Strong assessment, communication, and criticalthinking skills.

  • Ability to work independently in a fieldbased role.

  • Reliable transportation and valid driver's license.
