AT HOME CARE PHYSICIANS PLLC is looking for a compassionate and dedicated House Call Physician to join our team in Warrenville, IL. Responsibilities:
  • Conduct comprehensive in-home medical evaluations, diagnoses, and treatment for a diverse patient population
  • Develop and manage individualized care plans tailored to each patient's needs and medical history
  • Coordinate with specialists, home health agencies, and care teams to ensure continuity of care
  • Perform medication management, chronic disease management, and preventive care during home visits
  • Maintain accurate and timely clinical documentation in compliance with regulatory standards
  • Communicate effectively with patients, families, and caregivers regarding treatment plans and health goals
  • Respond to urgent patient needs and provide appropriate follow-up care
Requirements:
  • MD or DO degree from an accredited medical school
  • Active and unrestricted Illinois medical license
  • Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatrics
  • Valid DEA license and ability to prescribe medications
  • Experience in primary care, geriatrics, or home-based medicine preferred
  • Strong clinical judgment with the ability to work independently in a field-based setting
