Established in 2012 with the creation of a Medicare-certified agency in Southwest Florida, Discovery At Home has grown steadily over the years, expanding its operations by starting and acquiring agencies in Tampa, Florida; Plano, Texas; Houston, Texas; and West Palm Beach, Florida. While DAH primarily serves residents living in communities managed by the Discovery Senior Living family of companies, its services extend beyond these communities. Position Summary

Discovery At Home is seeking an experienced Registered Nurse Administrator with an entrepreneurial mindset to lead the clinical, operational, financial, and growth activities of the home health agency.

This position is ideal for a hands-on leader who approaches the agency as if it were their own business-someone who can identify opportunities, build strong referral relationships, recruit and develop a high-performing team, drive census growth, and maintain exceptional clinical and regulatory standards.

The RN Administrator will have direct accountability for the agency's overall performance, including patient care, compliance, staffing, financial results, referral development, and market growth.

Duties and Responsibilities

  • Provides overall leadership and direction for the agency's clinical, operational, administrative, and financial activities.
  • Develops and executes strategies to increase referrals, admissions, patient census, and market presence.
  • Demonstrates an entrepreneurial and ownership-oriented approach by identifying growth opportunities and developing solutions to operational challenges.
  • Builds and maintains productive relationships with physicians, senior living communities, hospitals, rehabilitation facilities, and other referral partners.
  • Partners with sales and business-development leaders to establish growth goals and improve referral conversion.
  • Maintains accountability for agency financial performance, including revenue growth, labor management, productivity, expenses, and budget performance.
  • Recruits, develops, supervises, and retains qualified clinical and administrative team members.
  • Directs and evaluates professional and auxiliary personnel providing patient-care services.
  • Ensures that staffing levels and clinical capacity support current and anticipated agency growth.
  • Oversees the planning, coordination, delivery, and evaluation of home health services.
  • Ensures compliance with all applicable federal and state regulations, Medicare Conditions of Participation, licensure requirements, accreditation standards, and company policies.
  • Develops, implements, and evaluates agency policies, procedures, clinical practices, and administrative systems.
  • Oversees patient-care services, including regulatory licensure, certification requirements, and accreditation standards.
  • Participates in the review and evaluation of the agency's overall effectiveness, quality outcomes, and patient experience.
  • Oversees Quality Assessment and Performance Improvement activities and ensures the timely implementation of corrective action plans.
  • Collaborates with physicians and other healthcare professionals regarding patient-care services.
  • Promotes a culture of accountability, urgency, collaboration, service excellence, and continuous improvement.
  • Represents Discovery At Home professionally within the organization and the surrounding healthcare community.

Qualifications

Required

  • Current, unrestricted Registered Nurse license in the state of Texas.
  • Previous leadership, administrative, or supervisory experience in a Medicare-certified home health agency.
  • Strong knowledge of Medicare Conditions of Participation, state home health regulations, accreditation requirements, and clinical compliance standards.
  • Demonstrated ability to lead teams, solve problems, manage competing priorities, and hold others accountable.
  • Demonstrated business acumen with the ability to understand agency financial performance, staffing productivity, referral trends, and operational metrics.
  • Strong communication, relationship-building, and organizational skills.
  • Entrepreneurial mindset with a demonstrated ability to build programs, develop teams, and drive sustainable growth.

Preferred

  • Three or more years of home health leadership experience.
  • Previous responsibility for agency growth, budgeting, financial performance, or profit-and-loss results.
  • Experience opening, expanding, or significantly growing a home health agency or healthcare service line.
  • Experience developing referral relationships within senior living, physician, hospital, or post-acute care markets.
