What are the key skills and qualifications needed to thrive as an at home health professional?

To thrive as an At Home Health professional, you need a background in nursing or allied health, relevant licensure or certification, and strong clinical assessment skills. Familiarity with home health care software, electronic health records (EHRs), and mobile documentation tools is typically required. Compassion, independence, and excellent communication are vital soft skills to effectively support patients in their own homes. These competencies ensure safe, high-quality care and build trust with patients and families in a less-supervised environment.

What is an at home health worker?

An At Home Health worker, also known as a home health aide, is a professional who provides personal care and assistance to individuals in their own homes. Their duties often include helping with daily activities such as bathing, dressing, meal preparation, and medication reminders. They play a crucial role in supporting elderly, disabled, or chronically ill patients who prefer to receive care at home rather than in a facility. At Home Health workers may also assist with light housekeeping and provide companionship. They are typically employed by home health agencies or may work independently.

What are some common challenges faced by at home health professionals, and how can they be addressed?

At Home Health professionals often encounter challenges such as managing diverse patient needs independently, adapting to varied home environments, and maintaining effective communication with remote teams. To address these, strong time management and organizational skills are essential, as is staying up-to-date with best practices in home health care. Building rapport with patients and their families, as well as utilizing digital tools for documentation and team collaboration, can also help ensure high-quality care and a supportive work environment.

Why Join Nurses At Home?

Nurses At Home is a growing home health agency with a fun, collaborative, and approachable company culture. We are committed to providing excellent patient care while creating a workplace where clinicians feel supported, appreciated, and excited about the future.

We offer:

  • Competitive compensation

  • Flexible part-time scheduling

  • The ability to build a schedule around your availability

  • A concentrated Waterbury-area territory

  • Supportive and accessible clinical leadership

  • Clinical independence and autonomy

  • Strong interdisciplinary collaboration

  • Opportunities for additional hours as the territory grows

  • Potential for future full-time or leadership opportunities

  • Mileage reimbursement

The Opportunity

As a Part-Time Home Health Occupational Therapist, you will help patients regain the skills and confidence needed to live safely and independently in their homes.

You will work closely with patients, families, nurses, physical therapists, physicians, and other members of the interdisciplinary team to address functional limitations following hospitalization, illness, injury, or a decline in condition.

This position is ideal for an OT who enjoys developing meaningful relationships with patients and providing individualized care in a one-on-one environment.

Responsibilities
  • Complete comprehensive occupational therapy evaluations and reassessments

  • Develop individualized plans of care based on each patient’s functional needs and personal goals

  • Provide skilled treatment focused on activities of daily living, upper-extremity function, strength, coordination, cognition, and safety

  • Assess home environments and recommend modifications, adaptive equipment, and assistive devices

  • Provide training related to bathing, dressing, toileting, meal preparation, and other daily activities

  • Educate patients, families, and caregivers on safety strategies and therapeutic exercises

  • Coordinate care with nursing, physical therapy, physicians, and other providers

  • Participate in interdisciplinary communication and care planning

  • Complete accurate and timely clinical documentation

  • Communicate changes in condition and safety concerns promptly

Qualifications
  • Current Connecticut Occupational Therapist license

  • Graduate of an accredited occupational therapy program

  • Valid driver’s license and reliable transportation

  • Strong communication, organization, and documentation skills

  • Ability to work independently while collaborating with the clinical team

  • A positive attitude and commitment to patient-centered care

Home health experience is preferred, but we welcome occupational therapists with strong clinical skills who are motivated to learn and grow in the home health setting.

Join a Company That Is Going Somewhere

You will have the opportunity to make an immediate impact, help strengthen our therapy program, and grow your role as our Waterbury territory expands.

Apply today and help patients throughout the Waterbury community regain their independence and remain safely at home.

Nurses At Home is an equal opportunity employer. We celebrate diversity and are committed to creating an inclusive and welcoming workplace for all employees.
