Job Specifics
Location: 1920 Libal St, Green Bay, WI 54301
FTE Status: Casual Part-Time, 0.40 FTE (16 hours/week)
Work Schedule: Day shifts between Monday & Friday, 8:00am-4:30pm; No weekends or holidays
Job Description:
Responsible for patient care under the direct supervision of a Registered Nurse. Responsibilities include:
  • Provides patient care according to Care Plan
  • Obtains vital signs
  • Collects patient data
  • Reinforces basic patient/family teaching
  • Assures patient/environment safety
  • Operates equipment
  • Performs documentation
  • Performs diagnostic testing
  • Completes annual education requirements
  • Provides age-appropriate care/service as applicable to role
  • Assures reliable, predictable outcomes and no needless waits
  • Uses Information Systems to their full capability
  • Duties as defined by specific department (if applicable)
  • Attends regularly scheduled and assigned shifts
Qualifications:
  • Listed on Wisconsin Nurse Aide Registry eligible to work in federally certified home health agencies.
  • Valid Wisconsin driver's license with auto liability insurance required.
  • Home health aide experience or at least one year of certified nursing assistant experience preferred.
