POSITION SUMMARY:

We are seeking a dedicated and compassionate Registered Nurse to join our team at Astoria Healthcare Center. As an RN, you will be responsible for providing direct, age-specific care to our residents, while also supervising and coordinating the care provided by other nursing staff. With a strong emphasis on teamwork and collaboration, you will work closely with physicians, consultants, and other healthcare professionals to ensure the best possible outcomes for our residents.

POSITION RELATIONSHIP:

As a member of our care team, you will collaborate with all levels of nursing personnel, administrative and financial staff, physicians, consultants, and other healthcare professionals to ensure the delivery of high-quality care.

QUALIFICATIONS/REQUIREMENTS:

  • Licensure/Certification: Current, valid Registered Nurse licensure to practice in the State of California and current BCLS certification

  • Experience: At least six months prior RN experience in a general acute care hospital setting, or acquired equivalent competency appropriate to the type of sub-acute residents that we provide care to

  • Language Skills: Ability to read, analyze, and interpret common scientific and technical information, and to be easily understood through verbal communication in the English language

  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals

SPECIFIC POSITION DUTIES:

  • Check daily staffing and assure staffing needs are addressed, updating schedules as needed

  • Supervise and administer direct nursing care when more than routine care is indicated

  • Make rounds twice each shift to assure proper care and a safe, clean resident environment

  • Meet with new admissions to assure core needs are being met

  • Assure accuracy and completeness of medical records

  • Review resident records for completeness and accuracy

  • Assist with admissions, resident care plans, and resident assessments

  • Perform assigned resident care duties in a manner that provides for the physical, psycho-social, and spiritual needs of our residents

  • Complete initial and ongoing assessments in a timely manner

  • Correctly differentiate between normal and abnormal clinical findings and intervene as necessary

  • Work collaboratively with the resident/family and interdisciplinary team members to develop an individualized plan of care for each resident

POSITION RESPONSIBILITIES/DUTIES:

  • Performs assigned resident care duties in a manner that provides for the physical, psycho-social, and spiritual needs of our residents

  • Completes initial and ongoing assessments in a timely manner

  • Correctly differentiates between normal and abnormal clinical findings and intervenes as necessary

  • Works collaboratively with the resident/family and interdisciplinary team members to develop an individualized plan of care for each resident

  • Demonstrates sound clinical judgment in the implementation and evaluation of the nursing aspects of interdisciplinary resident care plan

  • Directs and coordinates the care of each resident to meet their overall individualized needs

  • Provides treatment administration in a proficient manner

  • Provides and manages the care of technologically dependent residents

  • Identifies resident/family educational needs and initiates individualized teaching

  • Demonstrates knowledge of age-specific care and communicates effectively with residents, families, and colleagues

  • Reports and documents suspected abuse or neglect
