Job description

Alliant Health Solutions, a "Best Place to Work" and "Healthiest Employer", seeks a Social Worker, End Stage Renal Disease (ESRD)
The Social Worker is to mitigate grievances, appropriate interventions and access to treatment, drive interventions and provide technical assistance, to improve quality of life. Collaborate with other task orders developed for additional mental health initiatives. Support educational programs, quality improvement activities, community outreach, and response activities that encourage patient and family-centered care and engagement and to improve care for ESRD patients. Under the direction of the Executive Director and in collaboration with other departments, the social worker coordinates outreach through the provision of education, information, and training to ESRD professionals, patients, and their family members.
ESSENTIAL JOB FUNCTIONS:
  • Participates in the planning and evaluation process of projects.
  • Identifies patient educational materials focused on disease management and patient engagement to ensure appropriate health literacy.
  • Actively manages the grievance process to include adequate investigation and resolution.
  • Serves as a contact person for facility staff in addressing disruptive/abusive behaviors, at risk and involuntary discharges.
  • Coordinates educational webinars and materials for staff.
  • Brings a commitment of quality to all teams by identifying customer expectations, practicing process improvement tools and techniques, committing to continuous learning, communication, and teamwork development, analyzing problems, and offering innovative resolutions/approaches and building positive working relationships
  • Other tasks and responsibilities as assigned.
OTHER JOB FUNCTIONS:
  • Works in close collaboration with other team members to support the development of new projects and continuous improvement of the overall work process within the team.
  • Promotes core values of teamwork, professionalism, effective communication skills, and positive behavior.
  • Maintains security and confidentiality of all information in accordance with HIPAA laws, regulations, and company policies.
  • Demonstrates compliance with the corporate and departmental policies as evidenced by attendance, punctuality, and dress.

โ€ข Extensive knowledge and understanding of business practices associated with targeted provider settings. i.e., Dialysis Facilities, Hospital, Nursing Home, Home Health and Physician Office

โ€ข Knowledge, skill, and ability to work independently
โ€ข Knowledge, skill, and ability to collaborate with a team, work with self-directed teams, and manage a team
โ€ข Knowledge of and commitment to process improvement methodologies
โ€ข Knowledge, skill and ability to communicate in verbal and written formats, including ability to build relationships, present information, and facilitate meetings, including proven success with preparing and delivering presentations to both small and large audiences
โ€ข Ability to handle multiple activities/projects simultaneously and meet deadlines.
โ€ข Knowledge, skill, and ability to think critically and create solutions to challenges within ESRD
โ€ข Computer literate with Intermediate-Advanced knowledge of Microsoft Office, including Word, Outlook, PowerPoint, Excel, Teams, and Internet
โ€ข Ability to travel by driving or flying to Company locations, customer events or meetings or other locations as needed
EDUCATION, EXPERIENCE, AND TRAINING:
Required:
  • Master's degree, social work
  • Licensed social worker, preferably compact or in assigned states
  • 5 years' experience working in the ESRD community
  • Experience in social and behavioral sciences, including human factors discipline
Preferred:
  • Government or non-profit experience
