Whether working in hospitals, clinics, or long-term care facilities, RNs play a crucial role in coordinating patient care, ensuring safety, and improving health outcomes. RNs with specialization in Home Health and long-term care are highly valued for their ability to manage complex and critical situations.

Key Responsibilities:

  • Patient Assessment: Conduct thorough assessments of patients' physical and mental health status, including taking vital signs, reviewing medical history, and performing physical examinations.

  • Treatment Implementation: Provide and monitor treatments, including wound care, catheter care, and other medical interventions, ensuring adherence to established care plans.
  • Patient Education: Educate patients and their families about their medical conditions, treatment plans, and preventive health measures, empowering them to manage their health effectively.
  • Emotional Support: Offer emotional support to patients and their families, addressing concerns and providing reassurance during stressful or critical situations.
  • Care Coordination: Collaborate with physicians, specialists, and other healthcare professionals to coordinate comprehensive care, ensuring all aspects of patient health are addressed.
  • Critical Care Management: For those in specialized units such as ICUs or emergency rooms, manage critical care patients by monitoring life-support systems, responding to emergencies, and adjusting care plans as needed.
  • Documentation: Accurately document all aspects of patient care, including assessments, interventions, medication administration, and patient responses, in the electronic health record (EHR) system.
  • Quality and Safety: Adhere to all hospital policies, procedures, and regulations to maintain high standards of care, patient safety, and infection control.

Requirements:

  • Licensures: Must hold an active and unencumbered RN license in the state of Indiana.
  • Education: Associate's or Bachelor's degree in Nursing (BSN) from an accredited nursing program.
  • Certification: Basic Life Support (BLS) certification is required; Advanced Cardiac Life Support (ACLS) and other specialty certifications are preferred depending on the area of practice.
  • Experience: Previous experience in a clinical setting is preferred; specialized experience in Home Health or long-term care is highly desirable.
  • Skills: Strong clinical assessment and critical thinking skills. Proficient in medication administration and use of medical technology. Excellent communication and interpersonal skills. Ability to work in high-pressure environments and manage multiple priorities. Compassionate and patient-centered approach to care.
  • Physical Requirements: Ability to stand, walk, and move throughout the shift. Ability to lift, move, and position patients as needed. Ability to perform tasks requiring manual dexterity and physical endurance. Must be flexible to commute with in La Porte and Lake Porter Counties as needed.
