In your role as an LCSW, you will be tasked with the following key responsibilities:

• Conduct thorough psychosocial assessments to identify the emotional, social, and financial needs of pediatric patients and their families.
• Collaborate with interdisciplinary medical teams to create and implement individualized care plans, ensuring a holistic approach to treatment.
• Provide individual and group counseling sessions, focused on coping strategies, grief, and adjustment to health changes.
• Coordinate referrals to community resources and support services to facilitate care continuity after discharge.
• Offer educational resources and emotional support to families, guiding them through treatment options and available supports.
• Participate actively in hospital committees to contribute to policy development and improvement of social work practices.
• Deliver crisis intervention services when needed, providing immediate emotional support and coping strategies to patients and families.
• Maintain meticulous documentation in compliance with legal standards and organizational policies, ensuring all patient interactions are accurately recorded.

Your focus will primarily be on pediatric patients receiving hospice care, a sensitive and emotionally taxing environment that requires a high degree of empathy and understanding. You will work closely with families facing end-of-life challenges, providing crucial emotional and psychological support. Your role is instrumental in helping these families navigate their experiences, ensuring they feel supported, understood, and respected during profoundly difficult times.

The ideal candidate for this role will possess:

• A Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of pediatric healthcare dynamics and psychosocial issues.
• Exceptional communication and interpersonal skills, with the ability to establish rapport with diverse populations in challenging circumstances.

While not mandatory, the following qualifications are highly desirable:

• Two or more years of experience in a healthcare or hospital environment, particularly in pediatric or hospice settings.
• Familiarity with medical terminology and a background in mental health disorders.
