Registered Nurse (RN) โ€“ Home HealthPosition Summary

We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The RN will conduct comprehensive assessments, develop and manage individualized care plans, complete OASIS documentation, and collaborate with an interdisciplinary team to deliver high-quality, patient-centered care.
 

Key Responsibilities
  • Provide skilled nursing care and comprehensive patient assessments in the home setting.
  • Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, Follow-Up, and Discharge visits.
  • Develop, implement, and update individualized Plans of Care (POC).
  • Complete accurate and timely OASIS documentation in Homecare Homebase (HCHB).
  • Educate patients and caregivers on medications, disease management, home safety, and self-care.
  • Perform wound care, IV therapy, venipuncture, catheter care, pain management, and other skilled nursing procedures.
  • Coordinate care with physicians, therapists, case managers, and the interdisciplinary team.
  • Maintain compliance with Medicare, agency policies, infection control, and quality standards.
  • Participate in case conferences, team meetings, and on-call rotation as assigned.
Required Qualifications
  • Active Registered Nurse (RN) license in the practicing state.
  • 2+ years of Home Health nursing experience.
  • Homecare Homebase (HCHB) experience required.
  • OASIS experience required.
  • Current CPR/BLS certification.
  • Valid driver's license, reliable transportation, and auto insurance.
  • Associate Degree in Nursing (ADN) or Nursing Diploma from an accredited program.
Preferred Qualifications
  • Bachelor of Science in Nursing (BSN).
  • Certified Hospice and Palliative Nurse (CHPN) certification.
  • Strong clinical assessment, documentation, communication, and time management skills.
  • Ability to work independently while managing a home-based patient caseload.
